A History of Caring
Since 1981, Holy Family Nursing and Rehabilitation Center has served Des Plaines and surrounding communities. Whether someone is admitted for short-term rehabilitation or a long-term stay, an individually tailored program of care is developed for each resident.
The center is conveniently located at the corner of Dempster and Lyman Streets, just east of the Tri-State Tollway (I-294) in Des Plaines.

Holy Family Nursing and Rehabilitation Center offers comprehensive long-term and short-term skilled, intermediate and subacute nursing and rehabilitative care, including:

Meet the Administrator
Eileen Bregianos is the new Administrator at Holy Family Nursing and Rehabilitation Center. Ms. Bregianos comes to Resurrection Senior Services with extensive health care and senior services experience. Most recently, she was the Administrator of a 204-bed skilled nursing facility in the northwest suburbs where her responsibilities included all operational aspects. Prior to that, Ms. Bregianos worked at Friendship Village of Schaumburg from 2000 to 2005 in the following key management roles: Nursing Home Administrator; Home Health Director; Medical Clinic Director and Director of Nursing. She also worked at Saint Benedict Nursing and Rehabilitation Center as the Director of Nursing, and at Northwest Community Hospital and Continuing Care Center as a Clinical Nurse Consultant.
Ms. Bregianos is a licensed Nursing Home Administrator and a licensed registered nurse in the State of Illinois. She holds a Master of Business degree with a concentration in health administration from Keller Graduate School of Management and a Bachelor of Science degree in nursing from Northern Illinois University.
Licenses and Certifications
Holy Family Nursing and Rehabilitation Center is licensed by the State of Illinois Department of Public Health, and it is Medicare and Medicaid certified.
