The three-phase program includes:
- Phase I - This phase takes place after a cardiac episode while the patient is still in the hospital. During Phase I, the team develops an activity and education program to meet each patient's individual needs. Patients also learn about cardiovascular disease, medical procedures, risk factor modification and at-home care.
- Phase II - Next, is an outpatient program that is offered after a patient is discharged from the hospital. During Phase II, participants come to the hospital three times a week to work with the cardiac rehabilitation staff on an exercise and education program specific to their needs and interests. A cardiologist prescribes and assesses the participant's exercise plan through a portable heart monitor and blood pressure measurement.
- Phase III - This outpatient program is designed to maintain cardiovascular health and fitness. Participants learn to self-monitor their exercise programs under professional supervision.
