The Subacute Rehabilitation and Skilled Nursing Unit at Saint Joseph Hospital provides nursing care and rehabilitation services for patients recovering from stroke, respiratory illness, orthopedic surgery and other conditions. Short-term stays allow patients to transition more smoothly from an acute hospital setting to their homes.
In distinction from free-standing facilities, this hospital-based program provides patients immediate access to primary and consultant medical care, cardiac care unit, intensive care unit and acute hospital care 24 hours a day, 7 days a week. Physician specialists include board-certified geriatricians, physiatrists, plastic surgeons, vascular surgeons, podiatrists and infectious disease specialists who have advanced training in geriatric and wound care management.
The unit at Saint Joseph Hospital provides each patient with the encouragement, support and resources needed to progress toward recovery. Nurses on the Unit have special training in geriatric and rehabilitation nursing and are capable of caring for patients with demanding medical needs. A case management specialist regularly brings together the team to review the progress of patients and to supervise each patient’s care. Weekly conferences with patients and their family provide up-to-date information and exchange about patient goals, care and discharge plans.
The staff is a multidisciplinary team comprised of professionals from the following areas:
- Audiology (provides on-site hearing aid repair and sales)
- Dietician
- Geriatric and Rehabilitation Case Manager
- Occupational Therapy
- Pharmacists
- Physical Therapy
- Recreational Therapy
- Social Work
- Speech Pathology
- Spiritual Services staff
The Unit features the Sage Caregiver Training Room that provides a simulated home environment in which physical and occupational therapists work with patients, families and caregivers to prepare for a successful return home. Care for patients who require at least three hours of physical, occupational and/or speech therapy per day is provided in our Rehabilitation Inpatient Unit.
