This program brings independence, dignity and respect to chronically ill, homebound elderly people living in Chicago and its suburbs.

This non-profit program provides home health care for low-income, socially isolated and chronically ill elderly patients, while improving their quality of life and allowing them to remain in their homes.

This is the only program in Chicago that provides medical, nursing and social services regardless of the patient's ability to pay. The patients we serve cannot afford to pay for this service.

Your support is key to a successful partnership of patients, nurses, social workers, volunteers, families and donors which allows us to meet the needs of this population, in most cases, until the end of life.
