The Diabetes Self-Management Training Program at Our Lady of the Resurrection Medical Center provides the knowledge, skills and emotional support you will need to successfully manage your diabetes. The American Diabetes Association recognizes the hospital's Diabetes Self-Management Training Program as one that provides diabetic patients with high-quality self-management education services that meet the association's national standards.

Designing a Treatment Plan

There is no one "right" formula to managing your diabetes. Your treatment plan must be tailored to your own needs. Through the Diabetes Self-Management Training Program, our highly skilled multidisciplinary team can help you understand your treatment, including:

  • Nutrition - Registered dietitians provide personalized instruction on meal planning, portion control and weight management.
  • Medication - A certified diabetes educator explains oral medications and insulin administration.
  • Monitoring - A certified diabetes educator teaches the latest in blood-glucose monitoring technology and how to interpret results.
  • Exercise - You'll learn how getting the right exercise can help maintain proper blood sugar levels and help you feel better.

Education is Key

The more you understand about your diabetes, the better you will be able to take care of yourself. That's why the program's certified diabetes educator teaches basic facts about diabetes and provides information about possible long-term complications.

You also will learn why some patients require insulin while others can control their diabetes with oral agents. You will learn about insulin reactions vs. ketoacidosis with a focus on symptoms, treatment, causes and prevention. You will be taught how to monitor your blood glucose/urine ketones and how to control your diabetes during short-term illness. Preventive care for your feet, skin, eyes and mouth is also explained.

Emotional Support

Stress management and emotional adjustment are integral to the self-management program. Support groups meet monthly.

Registration and Referral

You will need a physician's referral to access diabetes and nutritional services. Your doctor need not be on the medical staff. The program's certified diabetes educator will contact your physician to secure the referral.
