    • Improving patient safety at home (as measured by improvement in ambulation and transferring). Our fall prevention and rehabilitation programs provided to our patients lead to high transferring and ambulation improvement scores as compared to other home health agencies.
      • PHC patients had 84% improvement in ambulation vs. 65% national database
    • Our standardized approach to managing chronic disease at home and our multidisciplinary care coordination leads to higher improvement scores in pain, medication management and dyspnea (shortness of breath).
