About Extended Hospital Care

Founded in 1961, Holy Family Medical Center continues its commitment to healing as one of the area's only specialty hospitals dedicated to providing advanced and specialized care for medically-complex patients.

As a specialty hospital for extended acute care, we serve patients who need hospitalization on average for about 25 days and in some cases even longer. We offer comprehensive care for patients with acute illnesses or serious injuries such as cardiopulmonary conditions, severe wounds, kidney diseases or neurological disorders such as head or spinal injuries or stroke.

Our interdisciplinary team of experts uses a wide range of specialized services and advanced technology to promote optimal recovery. Because we are focused on extended hospitalization, we are able to provide personalized attention in a close partnership with patients and their families. We understand the importance of treating every patient, not just as someone with a medical condition, but as a whole person with emotional, spiritual and social needs.

Holy Family Medical Center also offers a wide array of outpatient services. This means that patients can receive most tests and procedures on-site without having to be transported elsewhere. Located on a large, modern campus in Des Plaines, our beautiful facility includes a recently renovated lobby, a stunning chapel and comfortable rooms with many amenities.

Simple and Efficient Admissions

Admissions can take place seven days a week. The admissions process starts with the physician referring the patient, followed by an on-site evaluation performed by a nurse liaison. We also

encourage families to tour Holy Family Medical Center. Within 24 hours after the clinical evaluation, an admission decision is made. The admissions staff will assist and designate a date and time for the patient to be transferred and admitted. If Holy Family Medical Center is not the most appropriate health care setting for the patient, an alternative placement recommendation will be made. After the patient is admitted and the individual treatment plan is designed, an multi-disciplinary treatment team will meet regularly regarding each patient to ensure progress or modify treatment as needed to achieve the most favorable outcome. Our staff is always willing to answer any questions or assist in any way possible. Simply call 847-813-3330 to make a referral.
