PrimeCare Community Health Center - West Town
PrimeCare Community Health Center West Town is located in the Professional Office Building on the medical center's north campus. One of two continuity practices that residents may select upon entry to the program, the clinic is part of a federally-supported community health network operating on the city's near northwest side.
Like the Nazareth Family Center site located three blocks away, the center caters to a culturally and socioeconomically diverse, inner city community. With approximately 13,000 patient visits annually, the West Town practice offers residents with a rich setting to develop patient care skills, providing a well-balanced mix of obstetrical, pediatric, adult medical, and geriatric patients.
In addition to a broad scope of family medical care, other services include:
- Case management
- Health education
- Geriatric outreach
- Behavioral health
- Phlebotomy
The office is also a site for the National Diabetes Collaborative, a joint venture of the Centers for Disease Control and Prevention (CDC), the Bureau of Primary Health Care (BPHC), and the Institute for Healthcare Improvement (IHI). The Collaborative continuously monitors patient outcomes and allows residents to gauge their progress in achieving accepted standards of care.
The Office
The West Town office complex features:
- 11 exam rooms
- A procedure/minor surgery room
- Consultation room
- Resident and attending physician offices
- A small laboratory; and
- A remodeled library/conference area.

Reflecting the richness of the surrounding communities, the center serves a culturally diverse population. With a multilingual staff, residents and faculty provide comprehensive care to people of different backgrounds and needs.
As a Federally Qualified Health Center (FQHC), PrimeCare Community Health Center receives federal funds to assist in caring for all persons, regardless of their financial situation. This affiliation allows our physicians to fulfill the community service mission of the program. In addition, the clinic has contracts with numerous managed care organizations and commercial insurers. This enriches the diversity of patients seen in the practice and helps train residents to work in a variety of settings.
