About the Nazareth Family Center
The Nazareth Family Center (NFC), located on the South Campus (immediately adjacent to the main hospital), is one of the two continuity practices that our residents may select upon entry to the residency program. This site provides a "private group" model designed to prepare residents for future practice. Like the PrimeCare site located on the north campus of the medical center, which is three blocks away, the office caters to a culturally and socioeconomically diverse inner city community. With over 12,000 patient visits annually, it mirrors the kind of practice many residents may choose to enter on graduation.

- Case management
- Health education
- Geriatric outreach
- Behavioral health counseling
- Dermatology clinic
- Acupuncture
- Phlebotomy
The Office
The Nazareth Family Center features:
- 11 exam rooms
- A procedure/minor surgery room
- An acupunture room
- A separate library/consultation space
- A small laboratory
Faculty and residents work in teams to provide continuity to patients seen at the NFC.
Our Patients
The NFC has served the community for nearly four decades. It attracts patients from a variety of cultural and socioeconomic backgrounds, reflecting the richness of the inner city neighborhood. Nestled in the midst of one of Chicago's most diverse areas — bordering Wicker Park, Humboldt Park, Logan Square, Ukranian Village and Bucktown — the NFC boasts large Latin, Eastern European and African American populations, as well as newly-settled residents taking advantage of the clinic's proximity to the city's downtown business district.
