Do you have any of the following risk factors?

  • Pre-Diabetes
  • Impaired Fasting Glucose or Impaired Glucose Tolerance 
  • Family history of Diabetes
  • A parent, brother or sister that has diabetes
  • High blood pressure (blood pressure is 140/90 or higher)
  • High cholesterol
  • Overweight / Obesity + Inactivity (exercise less than 3 times per week)
  • Smoke tobacco regularly
  • Polycystic Ovary Syndrome
  • History of Gestational Diabetes
  • Given birth to at least one baby weighing 9 pounds or more
  • Family background is Alaska Native, American Indian, African American, Hispanic/Latino, Asian American, or Pacific Islander.

If you have at least one of these risk factors you could be in danger of falling onto the risky D-List. But we have great news for you! We can help you be an A-List success story for life. Register for our A-list diabetes prevention program.

What you receive free of cost:

  • One-on-one initial assessment with experts
  • 7 sessions with a Certified Diabetes Educator
  • 2 sessions with a registered dietitian and/or certified diabetes educator
  • Diabetes prevention education including nutrition counseling, information on exercise, problem solving, other lifestyles modifications, and how to monitor your health.

How to Register: Call the Community Health Department, Diabetes Nurse Educator, at 773.990.7755 for a pre-assessment. If you qualify, you will then be instructed to come in for lab work. Once we receive your results, we will contact you to come in for an initial assessment and discussion about your risk factors and your goals.

Classes held Thursdays from 6:00 - 7:30 p.m. starting November 8, 2018
